Who Can Apply
Caregiver requirements
- 18 years or older with a valid Social Security number
- Able to pass a criminal background check
- Reliable, patient, and compassionate
- Family members and friends of the client are encouraged to apply
- No prior professional experience required — we provide training through your caregiver portal
After you apply
You'll get access to the Caregiver Portal, where you can upload your ID and documents, enter your CHAMPS Provider ID, complete onboarding forms and training, see your assigned clients, and track credential expiration dates — all from your phone.
Enrolling as a paid family caregiver? The state form you'll need is the MSA-204 Caregiver Agreement — explained here. We'll help you complete it correctly the first time.
